Rungs, cleats, and steps must be parallel, level, and uniformly spaced (1926.1053(a)(2)). Every fixed-ladder rung or step must carry a single 250 lb concentrated load at its middle (1926.1053(a)(1)(iii)). Portable metal rungs, and fixed metal rungs made after March 15, 1991, must be slip-resistant (corrugated, knurled, dimpled, or coated). Ladders built and tested to Appendix A of Subpart X (the ANSI A14 series referenced there) are deemed to meet the load requirements.

Construction vs. general industry: 1926.1053 still accepts cages and wells for climbs of 24 ft or more. General industry (29 CFR 1910.28(b)(9)) requires a personal fall arrest or ladder safety system on fixed ladders installed on or after November 19, 2018, and on ALL fixed ladders over 24 ft by November 18, 2036 — cages alone no longer count there.

Setting up and using ladders the OSHA way

Ladders are Subpart X, not Subpart M — so the 6-foot fall protection trigger does not apply to someone climbing a portable ladder. The protection is in the setup: the 4:1 angle (base out one foot for every four feet of working length), side rails 3 feet above the landing you are stepping onto, level and stable footing, and a ladder rated for everything the worker and their tools weigh.

Load ratings: OSHA requires portable ladders to hold 4 times their maximum intended load (3.3 times for extra-heavy-duty Type 1A metal or plastic) and says never exceed the maximum intended load or the manufacturer's rating. In practice that means reading the duty-rating label — worker plus tools plus material. A big hand with a pouch, a drill, and a coil of wire can blow past a light-duty rating fast.

Fixed ladders on tanks, equipment, and structures trigger fall protection by climb length: 24 feet or more of climb needs a ladder safety device, an SRL with rest platforms, or a cage/well with offset 50-foot sections. Construction still accepts cages; general industry (1910.28) is moving every fixed ladder over 24 feet to fall arrest or ladder safety systems by November 18, 2036 — worth knowing if the ladder stays with the facility after turnover.

Near energized work, only nonconductive (fiberglass) side rails — 1926.1053(b)(12). Tag out damaged ladders the moment you find them, and have a competent person inspect periodically. FAQ

What is the OSHA 4 to 1 ladder rule?+

Under 29 CFR 1926.1053(b)(5)(i), a non-self-supporting ladder (like an extension ladder) must be set so the horizontal distance from the top support to the foot is about one-quarter of the working length — one foot out for every four feet up, roughly 75½ degrees from horizontal. Wood job-made ladders with spliced side rails use one-eighth instead (1926.1053(b)(5)(ii)).

How far must a ladder extend above the landing?+

At least 3 feet above the upper landing surface when the ladder is used to access it (1926.1053(b)(1)). If the ladder is too short to extend 3 feet, the top must be secured to a rigid support that will not deflect and a grasping device such as a grab rail must be provided.

What is the OSHA rung spacing for ladders?+

For portable and fixed ladders, 10 to 14 inches center to center (1926.1053(a)(3)(i)). Step stools are 8 to 12 inches; extension trestle ladders are 8 to 18 inches on the base section and 6 to 12 inches on the extension section. Minimum clear width is 11½ inches between rails on portable ladders and 16 inches on fixed ladders.

When does a fixed ladder need a cage or fall arrest?+

In construction, when the total climb is 24 feet or more (1926.1053(a)(19)): use a ladder safety device, a self-retracting lifeline with rest platforms at intervals not over 150 feet, or a cage or well with offset sections not over 50 feet and landing platforms at 50-foot maximum intervals. A shorter climb whose top is more than 24 feet above a lower level also needs a cage, well, ladder safety device, or SRL (1926.1053(a)(18)). General industry (1910.28) is phasing out cages in favor of fall arrest or ladder safety systems by November 18, 2036.

Can you stand on the top step of a stepladder?+

No. 1926.1053(b)(13) prohibits using the top or top step of a stepladder as a step, and (b)(14) prohibits climbing the rear cross-bracing unless the ladder is designed with steps on both sides. Stepladders also need a metal spreader or locking device (1926.1053(a)(8)).
