A scaffold inspection that actually means something
Scaffold citations are on OSHA's most-cited list every single year, and the causes don't change: missing planks, missing guardrails, no safe access, bad footing, no competent-person inspection. Most of them are visible from the ground in the first two minutes of a walk. The free Excel checklist above turns the before-shift inspection into a repeatable routine — and ties the result to the tag hanging at the ladder, where it matters.
What OSHA requires: 1926.451(f)(3)
"Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity." Three points to take from that sentence:
- Every shift, every scaffold. A tag signed Monday doesn't cover Wednesday.
- After any occurrence. Wind, a forklift strike, a freeze-thaw cycle under the base plates, a trade that pulled planks or a midrail to land material — each one triggers a new inspection.
- A competent person. Someone who can identify the hazards and has authority to correct them. Erecting, moving, dismantling and altering scaffolds also has to happen under a competent person's supervision (1926.451(f)(7)).
Walk it bottom to top
The checklist follows the order an experienced scaffold inspector walks: foundation first, then structure, platforms, guardrails, access, and use.
1. Foundation and base
- Legs on base plates and mudsills or other adequate firm foundation (1926.451(c)(2)).
- Footings level, sound and rigid — no settlement, washout, or thawing ground (c)(2)(i).
- No concrete blocks, bricks or buckets under legs or used as platforms (c)(2)(ii)–(iii).
- Screw jacks within the manufacturer's extension limit.
- Rolling towers: casters locked while stationary and stems pinned (1926.452(w)).
Most scaffold failures start at the ground. Check mudsills after every rain — especially on fresh backfill along a foundation wall.
2. Structure, ties and bracing
Legs plumb and braced against sway (1926.451(c)(3)); cross braces, pins and clamps in place; no mixed manufacturers' components unless they fit together without force (b)(10); nothing bent, cracked or corroded (f)(4). Then the 4:1 rule: once the height exceeds four times the narrowest base width, the scaffold must be guyed, tied or braced. The template calculates the ratio and tells you where the first tie goes and how often to repeat it — 20 feet for scaffolds 3 feet wide or less, 26 feet for wider scaffolds, at each end and at no more than 30-foot horizontal intervals.
3. Platforms and planking
- Fully planked or decked, with no more than 1 inch between units and at the uprights (1926.451(b)(1)).
- At least 18 inches wide for most scaffolds (b)(2).
- Front edge within 14 inches of the work, unless guardrails or personal fall arrest are used (b)(3).
- Planks extend at least 6 inches past the centerline of the support unless cleated or hooked (b)(4), and overhang no more than 12 inches for planks 10 feet or shorter, or 18 inches for longer planks (b)(5).
- Overlaps at least 12 inches and only over supports (b)(7).
- No splits, rot or overloading sag; wood planks not painted with opaque finishes that hide defects (b)(9).
- Clear of debris, snow, ice and slippery material (f)(8), (f)(13).
4. Guardrails, fall protection and falling objects
Anyone on a scaffold more than 10 feet above a lower level needs fall protection (1926.451(g)(1)) — guardrails, personal fall arrest, or both depending on the scaffold type. On supported scaffolds placed in service after January 1, 2000, the top rail is 38 to 45 inches above the platform; the midrail is roughly halfway. Cross bracing can serve as a midrail only if the crossing point is 20 to 30 inches above the platform. No steel or plastic banding as rails. Where people work or walk below, add toeboards at least 3-1/2 inches high, screens where material is piled above the toeboard, or barricade the area below (1926.451(h)).
5. Access
Once the platform is more than 2 feet above or below the point of access, provide a ladder, stair tower, ramp or equivalent — never the cross braces (1926.451(e)(1)). Hook-on and attachable ladders need their bottom rung within 24 inches of the supporting level and rest platforms every 35 feet (e)(2).
6. Use, loading and site conditions
- Not loaded beyond its maximum intended load or rated capacity (1926.451(f)(1)).
- No boxes, barrels or ladders on the platform to gain height (f)(14)–(15).
- Nobody on the scaffold while it's moved horizontally, unless the rules for mobile scaffolds are met (f)(5).
- No work in storms or high winds unless the competent person decides it's safe and workers are tied off or wind screens are used (f)(12).
- Power-line clearance maintained (f)(6).
The two calculations built into the template
Platform load vs. duty rating
Nonmandatory Appendix A to Subpart L gives the classic duty ratings: light duty 25 psf, medium duty 50 psf, heavy duty 75 psf over the span area. The template multiplies the duty rating by the bay's length and width, compares it with workers × your allowance per worker plus stored material, and flags an overload. Example: a medium-duty 7 × 5 ft bay is good for 1,750 lb uniformly distributed; two masons at 250 lb each plus 600 lb of block is 1,100 lb — 63%. Stack two more pallets of block on it and you're over. For system scaffolds, switch to "Manufacturer-rated" and enter the bay capacity from the manufacturer's data. It's a screening check — scaffolds are designed by a qualified person (1926.451(a)(6)).
Power-line clearance
Enter the line voltage and whether the line is insulated. The template returns the minimum clearance from 1926.451(f)(6) and compares it to your measured closest approach. Remember that the distance applies to the scaffold and anything conductive handled on it — rebar, metal studs, conduit, aluminum ladders. Green, yellow, red: tagging that tells the truth
Tags aren't an OSHA requirement on their own, but most owners and safety programs require them because they put the inspection result at the access point where the worker sees it. The template recommends a tag automatically:

The most common tag failure isn't the color — it's a stale tag. If the tag insert isn't signed for today's shift, treat the scaffold as not inspected.
How to use the Excel template
- Scaffold Inspection tab — one per scaffold per inspection. Fill in the info block and dimensions; gold cells are inputs, bold navy cells calculate.
- Read the auto checks — tie requirement, fall protection trigger, load check and power-line clearance.
- Answer all 38 items Yes / No / N/A, each with its OSHA reference. Write the corrective action next to any "No".
- Hang the recommended tag and sign.
- Weekly Log tab — one row per scaffold per inspection. It counts green, yellow and red tags for the week and flags any deficiency not yet corrected.
Common scaffold inspection mistakes
Signing the tag without walking the scaffold
A signature from the ground is not an inspection. Walk every level you can reach safely.
Missing the "after any occurrence" inspections
The trade that pulled a midrail to land material at 10 a.m. changed the scaffold. The inspection has to happen again before anyone else uses that level.
Mudsills on fresh backfill
Backfill settles, especially after rain. Check level at the base plates, not just at the top.
Ties removed by other trades
Window installers and siding crews take ties out to work. Relocating a tie is a scaffold alteration — it needs the competent person.
